I have a 82cm (32 inch) waist and I got a size medium. I only have 2-3 holes to make it bigger, but so far it's been great... Painful, but great. If I had the chance to reorder it I probably would have gotten a large just so I'd have room to bulk up a little. If only they had a belt in-between medium and large... That would have been perfect.
